This form should be completed to recommend book/print materials for LRC Selection. To mail the recommendation, press the Send Book Recommendation button at the bottom of the page.

In the appropriate spaces above, please enter the name of the faculty member making this library book purchase recommendation, the department or program in which they teach, and his/her e-mail address,. Then, in the spaces below, fill in all blanks and submit the completed form by sending it to the MLC (Send Recommendation button at bottom of page). Please use and send a separate form for each title recommended. If the professor making the recommendation is not the division/departmental allocation officer, the recommendation will be sent to the division/departmental allocation officer for his/her approval. This is done to assure coordination of purchases in each subject area. Then, the title will be entered into the MLC's order system.
