This form should be completed to recommend media materials for MLC purchase. To mail the recommendation, press the Send Media Recommendation button at the bottom of the page.

In the appropriate spaces above, please enter the name of the faculty member making this library media purchase recommendation, his/her e-mail address, and the department or program in which they teach. Then, in the spaces below, fill in all blanks and submit the completed form by sending it to the MLC (Send Recommendation button at bottom of page). Please use and send a separate form for each title recommended. If the professor making the recommendation is not the division/departmental allocation officer, the recommendation will be sent to the division/departmental allocation officer for his/her approval. This is done to assure coordination of purchases in each subject area. Then, the title will be entered into the MLC's order system.
